Our Services

  • Personal Care

    The term includes, but is not limited to, assistance with self-administered medications, feeding, oral, skin and mouth care, grooming, assistance with ambulation, bathing, hair care, dressing, toileting and transfer activities.

    • Performs duties as assigned by the client’s Care Plan.
    • Assists the client with activities of daily living including, but not limited to; bathing, grooming, dressing, toileting, oral hygiene, ambulation, and feeding.
    • Supports and provides a clean, safe, and healthy environment for clients and family members.
    • Performs housekeeping duties, such as: Clean/Sanitize kitchens, bathrooms, bedrooms, etc., Laundry
    • Assists in meeting the client’s nutritional needs by planning and preparing meals, and grocery shopping.
    • Assists the client with ambulation/transfers as specified in the Care Plan.
    • Reminds client to take self-administered medications.
    • Assists client with light exercise or movements in accordance with Care Plan.
  • Companionship Services

    Socialization, support and assistance with instrumental activities of daily living.

    Our companionship service provides general support for the non-medical needs of our clients in accordance with an established Care Plan.

    Support for the client includes but is not limited to: talking with the client about issues important or interesting to them; reading to a client, providing social and/or emotional support, letter writing, playing cards, and board games escorting the client to events or appointments and transporting the client to visit friends or relatives.

    • Supports and provides a clean, safe and healthy environment for clients and family members.
    • Performs housekeeping duties, including laundering and ironing.
    • May prepare and serve means as directed.
    • Ensures dishes are washed and the kitchen is clean and orderly.
    • Accompanies client on walks, errands, religious services, and events, as well as to the salon or other routine activities.
    • Reminds client to take self-administered medications.
  • Assistance with instrumental activities of daily living (ADLs)

    The term includes eating, drinking, meal preparation, ambulating, transferring in and out of a bed or chair, toileting, bladder and bowel management, personal hygiene, self-administering medication (medication reminders), light housekeeping, and proper turning and positioning in a bed or chair.

  • Dementia Care

    We can provide needed assistance for those living with dementia and Alzheimer’s.

  • Respite Care

    Personal care and assistance with instrumental activities of daily living provided on a short-term basis because of the absence or need for relief for those persons normally providing the services.

    • Meal preparation
    • Shopping and running errands
    • Housekeeping including, but not limited to:
      • Entire House: dust,vacuum floors and baseboards, clean hardwood and tile floors, wash windows and walls, take out all trash
      • Kitchen: sweep and mop the floors, clean and disinfect countertops/backsplash areas, clean the cabinets, appliances (fridge, oven, stovetop, range hood, inside the microwave, scrub the sinks, remove old food items from refrigerator, as directed by client
      • Bathrooms: clean/sanitize the showers/bathtubs, sinks, backsplashes and mirrors, clean/sanitize the toilets, inside and outside, mop floor/launder bathmats, polish chrome, empty wastebaskets, put out fresh towels
      • Bedrooms: dust and vacuum, change sheets and wash bedding if needed, make beds
      • Laundry: wash, fold, iron, and put away- clean laundry area
      • Plant care: water, dust, and prune
      • Pet care
  • Stroke Recovery Assistance

    We can provide needed assistance with activities of daily living, or respite care for a primary family caregiver. We can also help identify and eliminate hazards in the home that might cause or contribute to falls, which can prove devastating to the recovery process.

    The goals of rehabilitation are to help stroke survivors regain as much of their independence as possible. While many survivors begin their rehabilitation therapy in a hospital or rehabilitation facility, the vast majority of their recovery is performed over time at home.

    Rehabilitation nurses and therapists help patients who are able to perform progressively more complex and demanding tasks, such as bathing, dressing, and toileting, and they encourage patients to begin using their stroke impaired limbs while engaging those tasks. This is where our trained caregivers can be a vital link at continuing and enhancing the rehabilitation process.

    Let us help you and your loved one achieve your long term home therapy goals. We can provide needed assistance with activities of daily living, or respite care for a primary family caregiver. We can also help identify and eliminate hazards in the home that might cause or contribute to falls, which can prove devastating to the recovery process.

  • Senior day care facility aids

    We provide certified nurse assistants and home health aides to senior day care facilities on an outsource staffing basis.

    Some of these day care facilities are certified by the state to provide a variety of services to Medicaid recipients under the LIFE or PACE programs.  We are one of the few agencies that are able to do this sort of contract staffing since we have a lot of caregivers and a good electronic management system.

Community Health Choices (CHC)

The Department of Human Services (DHS) is committed to increasing opportunities for older Pennsylvanians and individuals with physical disabilities to remain in their homes. If you’re 21 or older and have both Medicare and Medicaid, or receive long-term supports through Medicaid because you need help with everyday personal tasks, you will be covered by Community HealthChoices (CHC).

OPTIONS – Provides assessment, care management and in-home services to aging and disabled adults who are experiencing some degree of frailty in their physical or mental health status. Consumers are over the age of 60.

LIFE ProgramAn option that allows the elderly to live independently on their own while receiving services and supports that meet the health and personal needs of the individual.

Hands From the Heart Home Healthcare Services currently provides contracted aides for several LIFE providers.

Hands From The Heart also contracts with long term insurance carriers and other third parties to cover the costs of services to its clients.  Please contact the office for information and how we can customize services to fit the unique needs of our clients.

Our services are paid for in a number of ways, including those eligible to receive benefits through one or more of the following government programs:

Aging Waiver – Provides long-term care services to qualified older Pennsylvanians living in their homes and communities.
Attendant Care / Act 150 – Information for mentally-alert Pennsylvanians with physical disabilities.
COMMCARE Waiver – Home and community-based program developed for individuals who experience a medically determinable diagnosis of traumatic brain injury.
Independence Waiver – Provides services to persons with physical disabilities to allow them to live in the community and remain as independent as possible.
OBRA Waiver – Provides services to persons with severe developmental physical disabilities, such as cerebral palsy, epilepsy or similar conditions.

Veterans Choice Program Hands from the Heart Home Healthcare Services is an approved home care provider through the Veterans’ Choice Program, we provide quality home care to our veterans who have served this nation proudly.

Aid & Attendance – This benefit can help cover the cost of in-home care for a veteran or their surviving spouse.

Homebound – A veteran must have a permanent disability evaluated as 100% disabling and due to such disability, he/she is permanently and substantially confined to their home.

Vet Assist Program – This program is for wartime Veterans, or their surviving spouses, who meet certain medical and financial requirements.  Once qualified there is no out of pocket cost for home care.
